    Please fill in and submit if you wish to purchase a UV device for personal or home use.  This will enable us to sell you the device at the discounted price.
    You should complete this declaration if you have a chronic medical condition and if the UV device you are purchasing is for your own personal or domestic use. 
    Please note: the definition of "chronic" is that the condition is constant, or recurs frequently. Vitiligo, psoriasis and autoimmune diseases are all defined as chronic conditions and entitle you to purchase your chosen therapeutic device without paying VAT, as long as the purchased item is for your personal or domestic use only.
